Requirements:
- Printed Application Form (will be printed at MB 121 upon submission of requirements)
- Original and Photocopy of PSA Birth Certificate
- Marriage Certificate (for married women only)
- Official Transcript of Records (with remarks "for further studies")
- Certificate of Employment (if employed)
- 4pcs 2x2 Colored Picture with white background (taken within the last six months)
- Long Brown Envelope
Note: A scanned copy of diploma may be used while your certified TOR is still in process. TOR shall be presented upon evaluation.
