Experience a seamless pre-enrollment process by following these simple steps!
STEP 1: LET’S CHECK!
VERIFICATION-MCC GATE (FRONT)
Upon arrival, kindly present your ID to the guard on duty to verify if you are on the list of qualified applicants. Once verified you will be given a CHECK LIST FORM. Fill out the form and proceed to the REGISTRAR’S OFFICE.
STEP 2: WELCOME!
Present your checklist form and submit your requirements for scanning and checking.
Do not forget to bring your complete documents.
FOR FRESHMEN
- Original Copy Form 138 (Grade 12 Senior High School Card
- Original Copy of PSA Birth Certificate
- Original Copy of Marriage Certificate (If married)
- Original Copy Barangay Certificate of Residency
- Original Copy Certificate of Good Moral Character (A or B Rating)
FOR TRANSFEREES
- Original Honorable Dismissal/Transfer Credential (Copy of Grades, Certificate of Eligibility to Transfer, and good moral)
- Original Copy of PSA Birth Certificate
- Original Copy of Marriage Certificate (If married)
- Original Copy Barangay Certificate of Residency
STEP 3: HANG IN THERE!
After submission of requirements and signed checklist from the Registrar’s Office, kindly proceed to the Finance Office (In front of the Registrar).
Mabalacat Resident - For the signing of the checklist
Non-Mabalacat Resident - For payment of Non-Residents fee
For pre-order and payment of lanyard and uniforms (P.E and NSTP uniforms)
STEP 4: LIGHTS, CAMERA, ACTION!
You’re almost done, the second to the last step is taking an ID photo and Account Creation at CISCO Lab - MB115.
STEP 5: LET’S CHECK YOUR MENTAL AND PHYSICAL HEALTH!
After at CISCO Lab, proceed to the Clinic and take your assessments.

STEP 6: CLAIM YOUR ID AND YOU’RE DONE!
CONGRATULATIONS! You are done with the PRE-ENROLLMENT Process. Kindly get your printed ID at the MIS Office and proceed to our exit.
Note: Please wear a white polo shirt or blouse with a collar, pants, and closed shoes.
